Premenstrual syndrome is a psycho-neuroendocrine (PNE) disorder characterized by physical, psychological and behavioural changes resulting in deterioration of interpersonal relationships and normal activity.

Etiology – unknown , Luteal phase condition

Premenstrual dysmorphic disorder (PMDD) is considered a severe form of PMS.

Stages – 1. Mild – Does not interfere with personal/social / professional life.

2. Moderate – Interferes with personal/ social/ professional life – still able to function and interact but suboptimally.

3. Severe – Unable to interact personal/social/professional life – withdraws from social and professional activities.

Clinical Features –

  1. Psychological – Irritability, tension, anxiety, mood swings, aggression, loss of concentration, depression, forgetfulness, mental confusion and fatigue, insomnia, change in libido, crying spells.
  2. Physiological – Bloating, weight gain, breast tenderness, headache, pelvic discomfort and pain, change in bowel habits, increased appetite, sugar cravings, generalized aches and pains, physical tiredness, weakness, clumsiness.

Diagnostic criteria –

  1. Not related to any organic lesion.
  2. Regularly occurs during luteal phase of each ovulatory menstrual cycle.
  3. Symptoms must be severe enough to disturb the lifestyle of the woman or she requires medical help.
  4. Symptom free during rest of the cycle.

Management – Non – pharmacological , Foods rich in B vitamins may help ( Thiamine and Riboflavin )

Homoeopathic Therapeutics – 

  1. Calcarea Carb – Menses – premature , profuse and protracted. Least excitement causes return of menses. Before menses – nervousness, nocturnal shivering, colicky pains, aching in back and hips, sensitiveness and swelling of mammae. During menses – Headache, Coldness of limbs, exhaustion, sour vomiting.
  2. Calcarea Phos – Menses – Too early, excessive and bright in girls. Before menses – Great sexual desire – copious flow. Headache – 4-7 days before menses. Griping and rumbling in bowels. During menses – Vertigo, throbbing headache, diarrhoea, no appetite, back pain, feels pulse all over.
  3. Cimicifuga – Menses – Early, profuse, dark and clotted. Before menses – Tenderness in hypogastrium, spasmodic uterine contractions. During menses – Shooting across bowels from side to side causing her to double up. Nervous headache. Menstrual colic from retroversion. Hysteric and epileptic spasms. After menses – debility.
  4. Cuprum metallicum – Menses – Too late , last too long. Before menses – dyspnoea, palpitations. Before and during – Severe cramps in abdomen radiating to chest. Nausea and vomiting. Convulsions of limbs, epileptic convulsions , shrieks in pain.
